About Your Visit

On your first visit please arrive 15 minutes prior to your appointment time and have the new patient forms that you were mailed or that you downloaded from this website, completed. If your forms are not completed, you will need to arrive 30 minutes before your appointment time so that you can complete the necessary new patient forms and your new patient chart can be assembled. If your insurance requires a referral please bring it with you to your appointment.

Also have the following items with you:

• Valid Drivers License or State ID
  
• Insurance card(s)
  
• List of medications you take on a regular basis, including over-the-counter
  medications and vitamins.
  
• List of surgeries or medical procedures you have had
  
• Payment for your co-pay or any non-covered services. For your convenience,
  we take cash, VISA, and MasterCard.
