It is with mixed emotion that I am writing this letter to you, I will be moving back to my home state of Missouri. My last day with Associates In Women’s Health Care will be on June 25, 2020. I am excited to start a new chapter and will reopen a new practice and continue with Bio-Identical Hormone Therapy via Telehealth and telephone in August 2020. 

To continue your BHRT treatments with my new practice, please email me at BurtonBHRT@gmail.com with the following information; Name, Date of Birth, Phone number.  I apologize for any inconvenience or delay this move may create for you, but I am committed to making the transition as simple as possible for you.

Associates In Women’s Health Care is staffed with some of the most knowledgeable and caring physicians in the country. I highly recommend that you select one of these Physicians or Nurse Practitioners for your future gynecological needs.

Your medical records are confidential and will remain on file with Associates In Women’s Health Care. If you choose another physician within AIWHC, you do not need to take any action concerning your records. If you choose a physician outside of Associates In Women’s Health Care or chose to continue your BHRT with my new practice, you will need to sign an authorization form before AIWHC can release your medical records. Please note, by law we cannot share your medical information without your written consent. You may sign this form at our office.
